The Integrated Care Strategy
The strategy sets out our aim to narrow the health gaps between the richest and poorest, enable every individual to live more of their life in a state of good health, and make sure people can access resources and services in their communities to manage their own health and wellbeing.
In delivering the strategy we are working to:
- Build strong communities and playing our part in creating opportunities to meaningfully involve and connect individuals
- Reduce the number of preventable deaths from heart attacks and strokes so people can live longer healthier lives
Social connectedness programme
Social connectedness can be defined as the experience of belonging to a social relationship. This programme takes a life course approach to improve social connectedness, reducing social isolation and loneliness and building social capital through supporting organisations, communities and individuals across Hampshire and the Isle of Wight.
The focus of this programme is:
- Creation of a social connected maturity tool to help measure the impact of social connectedness initiatives
- Supporting research to help understand social connectedness
- Promoting the importance of supporting social connectedness in the workplace
The Integrated Care Partnership held a social connectedness assembly in February 2024 which shared examples of good practice and research.
Prevention programme
This programme is focused on the prevention of cardiovascular disease, which is a major contributor to preventable ill health and deaths. The programme is intended to help increase healthy life expectancy, narrow the gap in health inequalities, reduce geographical variations in outcomes, address risk factors and identify those at risk earlier.
This focus of this programme is:
- Delivering a cardiovascular disease implementation plan
- Working with partners to address the risk factors of tobacco dependency, alcohol consumption and lack of physical activity
