The service provider / Nursing Home is required to notify the Continuing Healthcare team when the client’s circumstances change. Notification must be given if:
- the client dies
- the client is absent from the Care Home due to an admission to hospital
- the client returns to the Care Home after a hospital admission
- the client leaves the Care Home on a permanent basis for any other reason.
Important:
- Please do not use this form for clients living in Southampton — there is a separate form available here.
- For Isle of Wight and Portsmouth clients, please continue to follow existing processes.
