Our GP practices are now home to Digital Care Coordinators who help patients get set up online and on the NHS app. They can help patients install and use digital tools such as the NHS app and help them navigate the many features that are available.
Across Hampshire and the Isle of Wight more than 62 per cent of residents are using the NHS App to order prescriptions, make appointments and view their test results. It is a great resource that gives residents a simple and secure way to access a range of NHS services at their fingertips.
Kay, a Digital Care Coordinator at a GP practice in Hampshire, said: “I have been working as a Digital Care Coordinator over the last year, and I really enjoy working with our patients to ensure that they get the best out of the NHS app and are able to use all of its features.
“There are so many people in our community who struggle with technology and my role allows me to show them how to use the app to order repeat prescriptions, book appointments, view their health records and many other things. I see patients both at the practice and in their own homes helping them to realise the benefits of being able to log in and use the app to help with their medical care.”
The NHS app allows patients to view their GP health record and helps people to better manage their condition without having to phone their GP surgery. People can also access consultation notes, test results and future appointments.
Patients are being encouraged to switch on their notifications within the app, to ensure that they receive messages directly from their GP practice. Instructions on how to turn on notifications can be found here: Managing NHS App notifications
As part of the NHS 10-year health plan, there is a drive to ensure that patients have access to care within their communities. Local people across Hampshire and the Isle of Wight are now able to book more appointments with different healthcare professionals at GP practices, meaning they can be seen quicker and cared for in the best possible way.
Our Digital Care Coordinators are also supporting the 10-year health plan objective to move from analogue to digital by working within our primary care teams to help those who are digitally excluded.
In March 2025, NHS Hampshire and Isle of Wight launched the ‘It Takes a Team’ public information campaign to highlight the different specialists working within GP practices. The 12-month campaign will throw a spotlight on a different specialist role each month and will feature people working within those roles in our local practices. This month we are focussing on our Digital Care Coordinators.
James Roach, Director of Primary Care and Local Care at NHS Hampshire and Isle of Wight said: "Our Digital Care Coordinators are becoming an integral part of our GP practices supporting those in our communities who have struggled to get online and use the services available via the NHS app.
“Our GPs are seeing record numbers of patients each week and by having specialists like Kay in our practices, it ensures that patients can use the NHS app to keep themselves up to date regarding their medical treatment at their own convenience in their own home.”
