In order to ensure that Funded Nursing Care (FNC) applications are processed as quickly and efficiently as possible, please submit an FNC admission form within 28 days of a new resident’s admission. If we do not receive the FNC form within 28 days, we will only pay FNC from the date the form is received.
If you wish to make an FNC application for someone in receipt of S117 aftercare please contact us to discuss before completing this form.

- Please do not use this form for clients living in Southampton — there is a separate form available here.
- For Isle of Wight and Portsmouth clients, please continue to follow existing processes.
