We are committed to working together as health and care partners in Hampshire and the Isle of Wight to improve the cardiovascular health of our communities.

We know that cardiovascular disease, heart failure, diabetes, stroke and chronic kidney disease are the leading causes of preventable ill health and death in our communities. This is driven by risk factors including hypertension (high blood pressure), smoking, obesity, and limited access to prevention services.

At our recent ‘signature move’ event we came together to agree a joint approach to cardiovascular health and the concrete actions we can take to benefit patients and communites. You can read the detailed outputs from the day below. This is our plan to work together as one system to improve patient outcomes, address inequalities and place a greater focus on prevention.
